Gout Associated with Recurrent Aggressive Chronic Lymphocytic Leukemia: A Case Report

Michael W. Downey*, Paul M. Ryan, Jerome J. Wenninger Jr

A patient with a positive history of extensive Chronic Lymphocytic Leukemia and previous CLL treatment presents with a right  foot painless mass. Due to the patient’s history and an increase in serum uric acid, tumor lysis syndrome as well as severe  hematological abnormality could not be ruled out.

The Use of Anti Citrullinated Protein / Peptide Antibody Assay as Diagnostic Test in Patients with  Rheumathoid Arthritis

 Spasovski Dejan*, Tatjana Sotirova

Rheumatoid arthritis (RA) is an autoimmune disease, multi-functional in origin, characterised by the inflammation of the membrane lining joints. The disease spreads from small to large joints, with the greatest damage in the early phase. The diagnostics of RA is based on clinical, radiological and immunological features.

Case Report: Histologic Study of Marlex Mesh Used in the Reconstruction of the Extensor Mechanism of the Knee at 4 Months After Implantation

Donald Hohman, M.D*, Daniel Donovan, Lucia Balos, M.D., Sridhar Rachala, M.D.

Disruption of the extensor mechanism of the knee is a rare but potentially disastrous complication of total knee arthroplasty  (TKA) with few reliable options for repair or reconstruction. Of growing interest is the use of synthetic mesh to reconstruct   a ruptured extensor mechanism. To our knowledge, crystalline polypropylene (Marlex) mesh used to reconstruct the extensor mechanism of the knee has not been described histologically.

The Value of SPECT/CT in the Diagnostic Process of Chronic Non-Specific Wrist Pain

Inga S. Besmens, MD*, Frank G. Fuechsel, MD, Corinne Chmiel, MD, Catherine Hess, MD and Christoph Meier, MD

Chronic non-specific wrist pain is a diagnostic challenge. The purpose of this study was to evaluate the benefit of 3D-scintigraphy (Single Photon Emission Tomography (SPECT)) combined with high resolution computed tomography (CT)  compared to established standard imaging modalities in a clinical workup. Methods: From March 2010 to March 2013, 36  consecutive patients with chronic nonspecific wrist pain undergoing SPECT/CT were reviewed retrospectively. All patients were  initially evaluated by non-conclusive standard imaging modalities, such as bi-/multi-planar xrays of the  wrist (n=36),  CT (n=2), sonography (n=17), MRI (n=10), MR arthrography (n=7), conventional scintigraphy (n=2) or diagnostic arthroscopy (n=2), before SPECT/CT was employed.

Some Methabolitic Aspect and Described Determination with Explaination of Influence of Prognostic Factor between Linked Overall Derivative of Dimethylarginine and Acpa Antibodies in Patients with Rheumatoid Arthritis

Dejan Spasovski*, Tatjana Sotirova

The association between arginine and citrulline, i.e., between asymmetric dimethylarginine (ADMA) as dimethyl derivative  of the amino acid L-arginine and anti-cyclic citrullinated peptide antibodies (Anti-CCP2) of the second generation in patients with Rheumatoid arthritis (RA) is the isoform of the enzyme peptidylarginine deiminase (PAD). Enzymatic deimination or protein  citrullination is a process catalyzed by PAD enzymes which take part in the pathogenesis of RA.
